Login to user account using instructions from "How to Login". If you do not currently have an account with the website, please follow the instructions regarding how to "Register a New Account". |

Click the orange "--> Forms" link in the drop down menu at the top of the page under "Forms". |

Locate the ICJ form to download. The ICJ Forms are available in Microsoft Word and Adobe PDF versions. Click on the *.doc link for the Microsoft Word version or the *.pdf link for the Adobe PDF version.

The form should open in another window or tab. Click "File" and then "Save". Select a folder on your computer to save the form in and click "OK". This step may vary for you depending on your internet browser settings. |
